Bunions are probably the most common foot disorder seen in podiatry. The term bunion itself is used by patients describing the bony lump found near the base of the big toe, which is usually an adaptation of the positional change of the big toe. Hallux abducto valgus (HAV) is a medical term, which describes the position of the hallux (big toe) with respect to the connecting bone of the mid foot (metatarsal). In this foot disorder, the hallux deviates towards the lesser toes and the metatarsal moves towards the midline.
There is much debate as to which is the major cause, but it is likely that your genetic makeup makes you more prone to a bunion or bunionette and that then wearing ill-fitting footwear causes them to develop. Studies have shown that in cultures where people don?t wear shoes but are habitually barefoot, there are very few cases of foot bunions indicating a strong correlation with shoe wear. They are more common in females, most likely due to choice of footwear.
The symptoms of bunions include irritated skin around the bunion, pain when walking, joint redness and pain, and possible shift of the big toe toward the other toes. Blisters may form more easily around the site of the bunion as well. Having bunions can also make it more difficult to find shoes that fit properly; bunions may force a person to have to buy a larger size shoe to accommodate the width the bunion creates. When bunion deformity becomes severe enough, the foot can hurt in different places even without the constriction of shoes because it then becomes a mechanical function problem of the forefoot.
Diagnosis begins with a careful history and physical examination by your doctor. This will usually include a discussion about shoe wear and the importance of shoes in the development and treatment of the condition. X-rays will probably be suggested. This allows your doctor to measure several important angles made by the bones of the feet to help determine the appropriate treatment.
Non Surgical Treatment
Treatment may be surgical or non-surgical. The goal of non-surgical treatment is to relieve pressure on the foot and to prevent pressure sores and foot ulcers. This is accomplished by prescribing accommodative shoes with a wide toe box - sandals or extra depth shoes with soft moulded insoles. It may also be possible to relax the leather on shoes to make room for a bunion.
Anyone who experiences symptoms from bunions should see a podiatrist for treatment. But you may benefit from surgery if you have any of the following. Severe foot pain that limits your everyday activities, including walking and wearing comfortable shoes. You may find it hard to walk more than a few blocks (even in athletic shoes) without significant pain. Chronic big toe inflammation and swelling that doesn?t improve with rest or medications. Toe deformity, a drifting of your big toe toward the small toes. Toe stiffness, inability to bend and straighten your toes. Failure to obtain pain relief from anti-inflammatory drugs. Failure to substantially improve with other treatments such as a change in shoes and anti-inflammatory medications.
To help prevent bunions be sure your shoes don't cramp or irritate your toes. Choose shoes with a wide toe box - there should be space between the tip of your longest toe and the end of the shoe. Your shoes should conform to the shape of your feet without squeezing or pressing any part of your foot. Avoid pointy-toed shoes.
